Jeff,
A lot of accidents happen when pro's who have done something for years get complacent and try to push the envelope a bit. Whether its winches, table saws, chainsaws whatever.
I have to unzip my fly when I need to count to ten after a tussle with a table saw in 95. I find I am much safer now than I was then and had been using saws for 20 years prior to the accident.
