My tractor did the same thing that you mentioned. You could see bubbles and exhaust coming out of the radiator cap hole.
He could also fire it up cold with the cap on and run it for maybe 30seconds to 60 seconds and shut off. Open the cap and if he has pressure then theres a good chance he's getting exhaust in the cooling system.
It also was a one way leak. Never did get any coolant in the cylinder but it sure would send exhaust the other way.
