Once the engine gets sufficiently worn out, and you're putting in a quart every couple hundred miles, it's easy to convince yourself that there's no point in changing the filter with all that fresh oil going in all the time. It's sort of a "flow-through" lubrication system -- one pass through the bearings and then across the cylinder walls and out the tailpipe.
