Rods often have a squirt hole to lube the cam, and may be offset (fersure on 230), so shouldn't be flipped. Besides, flipping a rod wouldn't do anything since two holes are symmetric.
But pistons have the wrist pin hole off center to allow the piston to stay rocked to one side, to quiet any slap. But then, the way the flat-head combustion chamber is off to one side, turning them 180 may not be desirable- no matter the rotation, the expansion pulse still hits the same side first.
So maybe on overhead valve engines you would turn pistons, but not if there is an interference with valves. Like BB Chevys or Hemies.
