W250 warped rotor help

My truck keeps warping brake rotors. Every few thousand miles, the shakes come back. Can anybody suggest a solution?

Are the rear brakes not staying adjusted and causing the front to get too hot?

Are the premium, long wear, anti-squeak pads not disipating heat properly?

Are the replacement rotors junk?
