Paper ID tag for 57 PW, Got one?

Both my W-100 and my FFPW are 57s. Both came originally with the paper ID tags glues into the glovebox. Brilliant idea on someones part. Both of my trucks and 99% of the trucks that had the paper tags are missing them.
I would be interested in trying to reproduce them for myself, and for others if they were interested, but I do not have one for a pattern. Anyone have one they would loan me, even a photocopy would work.
Or, if someone knows of them being available, there is no need for me to re-invent the wheel.
