Skyjacker's lift was 6", I'm not sure they still carry it.
National Spring in San Diego may custom build you a lift. I'd say that 4-5" for a '64 would be more than plenty. These trucks will clear 35's just fine in stock form (slight rubbing at the front lips of the front fender wells, at full compression, when turning).
A 12" lift would be ridiculous and limit the type of terrain that you can traverse.
