When going down hill the truck is driving the tranny and the load is placed on the opposite side of the gears. When you begin driving again the load now shifts to the driven side of the gear.

Why only in forth gear? Is it rpm or gear related? Does the forth gear show abnormal wear in the tooth pattern to set up this harmonic or is it from a bearing now turning at a highr RPM? My experience has been that a worn pilot bushing usually makes noise in all gears.

If you can not determine the source with the top of the tranny off then the next step is to remove it. The noise you discribe is not normal.
