The big pump diaphram has the fuel on one side (of course), the other side is exposed to the crankcase. If this diaphram tears or gets a hole, it will pump fuel out under pressure into the crankcase. There are only a few fuel pumps made that have isolated diaphrams on them. Most older vehicles have diaphrams exposed to the crankcase, so it can happen on just about any old vehicle. I had a 390 Ford do that once. Filled 'er right up.
