Seems to me the hardest question is how do you drive two rear axles. In military trucks, the GMC approach was to drive each rear axle from a separate driveline coming out of a transfer case with two rear outputs, while the Reo approach was to equip each differential with a straight-through driveshaft with an output flange. The only one that gets used is the one on the front rear axle, but they all have it. Neither approach would be easy for a one-off conversion of a modern truck. It wouldn't be hard to have a tag axle in the rear, but two driven axles seems like it would require an extra gearbox unless there's some three-output transfer case I'm not aware of.
