Pilot bushings provide good service, and have in many trucks of all kinds and sizes. I would never be afraid of one.

Having said that, if you really wanted to keep a ball bearing type pilot, and could not find one to bridge that big range of dimension between crank bore and pilot diameters, I would get a ball bearing pilot bearing that fit the pilot part of the input shaft, and if the OD was too small, look at bushing that side. Meaning, install a bushing that is an interference fit in the crank bore, that then accepts your new ball bearing.
