the dowel pin on the inner nut faces OUT, allowing it to engage with a hole in the washer.
adjust the wheel bearing, then try the washer, flipping it both ways to see which way fits better. if adjustment of the inner nut is required, only loosen, not tighten, to align the dowel with a hole.
the washer, which engages with a keyway, by virtue of the internal tab, then prevents the inner nut from turning. the outer nut serves as a jam nut, securing all this.
some trucks you take apart will have the inner nut arranged correctly, but the repairer did not get a hole in the washer over the dowel. you can tell this by the horrendous dent in the washer.
