I am using a material called "Armaflex", which is a high density foam rubber used in heavy industry, and is available in sheets of varying thicknesses. You could use a half-inch thick layer for the floor and a two inch thick piece for the shifter penetrations, with a piece of carpet on top of it to add durability.
The noise reduction is significant but bear in mind that the moisture retaining qualities of foam may exclude this material from consideration, depending on your climate and on how much your cab leaks.
I glued several sheets of 2" armaflex to the inside roof of my cab and can attest to the isulative properties and noise reduction benefits.
