Fumes coming out the road draft tubes is piston ring blow-by usually on a PW caused by spending much of its life sitting in a field & getting rusty cylinders.
If you do a ring job you'll likely cure the problem but as David said, often this is a sign of condition of the rest of the engine.
Rings are cheap if you can do them yourself in the truck...it can't hurt but you'd be well of measuring bearings & checking the condition of the rest of the engine.
Have you done a compression test on the engine?
