If you can, do a compression test on each cylinder. This will tell you whether you have a cylinder with bad rings or a hole in the piston. If the compression checks out good, then it's something like Joe was saying, fuel leaking at any one several different points in the fuel system, i.e. the pump, fuel lines, at injectors, and don't forget the fuel return lines to.
