Cam,
I was thinking the same thing for my van...tranny. We had two clutch packs (2nd and 4th) replaced under warranty for shudder on shift at ~26k miles, so I was scared.
Our shudder was during acceleration or under load. Back off the gas and it would go away, step on the gas and it came back. Get over ~30mph and it seemed to go away unless you were going up hill. When it firt started it felt like going over rumble strips on the highway but progressed to the point that it shook the front-end pretty bad (felt like a bad tire out of balance).
Mechanic friend suggested engine mounts. It didn't eliminate the shudder but it did move the speed/RPMs. Ended up being the right axle shaft. Replaced both for good measure and all has been good for ~10k miles....knocking on wood.
We bought our '96 T&C in May of '97 with 18k miles on the clock. It now has over 121k on it; still runs great, still gets ~17mpg in town (20mpg highway) and my bride still enjoys driving it! (Which is a good thing.)
Good luck!
