I've been looking at cabs in particular with this very question in mind. From what I've found bare / primed, painted I'd guess that the cab was assembled in bare steel, and then sprayed with primer (not too efficiently) Firewall pad seems to have been put in after paint on civilian trucks but before paint on some army cabs.
Primer normally visible behind kick panels, under dash and cowl vent, behind headliner but not behind seat.
