I have a 91 D250 that I bought new and just past a 100K. The only thing I have spent money on is a throttle position sensor cause it wouldn't stay in od. I can get 18 to 20 mpg around town and up to 24 on the road . A close friend smashed his 92 W250 and could wait on a dealer to find a new Dodge so he settled for brand F. With the Goat he could haul 4 horses from here to Colorado and get 19.6 mpg. With his 97 PS he sometimes gets 17 empty and needless to say he is anxious to get the Dodge rebuilt and sell the Ford. The answer is yes they will get 20 or more mpg and don't let miles booger you cause I have seen those things with way over 600K and still don't use oil.
