Message:   Without seeing the mix design, I can't say if you got stroked or not, but a 5,000 psi mix would not be possible without 1 1/2" aggregate AND about 8 sacks per yard, at a 2" slump. I've placed 4,800 psi mixes for defense work and they are NOT your ordinary run of the mill mix designs.
At 4" thick, it would be nearly impossible to say that the slab is that strong, as thickness is also a factor in load the bearing capacity of a slab. 4" slab on grade is for tract houses and sidewalks, 5" over 2-3" base is starting to get serious and 6" over 6" base is a good strong shop slab for heavy trucks. Roadways that see truck usage out here get paving that is about 9" thick, placed upon an 12-15" base.
