The yoke was probably drilled on a Monday or Friday. If you haven't, try rotating the yoke 90 degrees for a better line-up. If thats isn't any any better, you may need to drill to match or new holes.
The 3/16 plate was probably used to adjust the worm bearings, but taken off during a readjustment? The gaskets are 1/16 think that are used.
