This is the correct tailgate for your application, and it includes the side hinges, no center hinge, but you can get repo center hinges from VPW, or I can get a used one.

There are better tailgates out there, but there are also much worse ones. This one is pretty straight, and only needs a peice of pipe driven into the lower tube, replacing the center pivot, to be used.
