It is amazing how the obvious is regularly overlooked. Go ahead and check all the other hubs ASAP as they may have the same problems. Especially because of the missing nut and poor nut adjustment. I would assume they are the same or worse. If they are ok you have peace of mind. I also note the chisel marks on your hub nut which I have used to both remove and tighten them for many years but I chipped out the $20 or so for the hub nut socket which I assume you did as well but if not get one and it will be right. So much better than a chisel. Those bearings are getting very expensive.
