You still have to be a pretty good aim with those things. It is basically a "V" made of plate metal that guides the coupler tongue over the ball to help in lining it up.
While I don't doubt they help - I've found that an angled mirror temporarily mounted on the trailer or on the back of the truck allows you to see the ball/coupler and helps in lining them up.
