Depending on which manual you look at, here is the order:
1 - Under the brake band cover (thin)
1 - under the rear bearing retainer retainer (thick)
2 - under the front bearing retainer (thick/thin)
Adjustment to the worn is made by adding/subtracting gaskets at the front bearing retainer. You should be able to turn the worm shaft by hand w/o any bearing side-to-side or end play. You may have to shift gaskets around to get the desired adjustment.
