If the D shaped piece shown turns, and will help remove the bolt, try this:
Drill a hole in each side, enough diameter to insert a strong dowel, or better yet, a couple of pin punches. Stand them up vertically with the tips into the holes you drilled. Next, take a long screwdriver or similar rod, and slide it between the two punches to act as a lever. Use this setup as a sort of spanner wrench to turn the D shaped part shown.
