M37 starter pinion...

Often when I hit the starter (above the accelerator pedal) I get a whirr but no engagement. A tap or two later, the pinion will engage the flywheel and we're good to go.
Is this just poor lubrication on the bendix, missing teeth on the pinion, or an alignment problem. I know some other starters require shims and a lot of old ones are simple bolt ups…I'm pretty sure it's not a voltage problem.
