Lean manufacturing has cost companies millions of dollars from overnight shipping and paying double or triple for things to be made on overtime. Trust me, it happens all the time at or company and we have to rush, work 7 days to fill the need it now orders. The only plus side is that is has ironically kept some jobs in the USA. They can't get the stuff made and shipped to them in time when like you said, a line or machine is down. It all stems from big business where the unneeded costs would have not been tolerated by an owner who was hands on. Now it gets buried in "normal" costs by the bean counters.

I will say this. I used to think the US was doomed from Japanese companies and how they would cut costs and streamline things. My opinion has changed since I have had the first hand experience of dealing with some. They are worse than anything I have ever seen. They are great at making a billion widgits but custom stuff/small quanities they can't grasp.
