It appears that they tightened the inner nut and lock ring, but couldn't or didn't get the outer nut on very well. It was super tight with only 2 thread engaged on the spindle. It doesn't appear to be cross threaded. The nose of the outer nut was sticking out and rubbing the back of the hub cover, enough so that it blistered the paint on the cover. I think the hammering mushroomed the outer 1/4" of the spindle. But all this is just a guess.
