It might be the markings aren't matched with the chain case cover. I've seen chain case covers that have the pointer at the 12:00 position and ones that have it 5 or 7 degrees before, so if the cover and crank didn't come as a pair....

My VC3 had a crank that was at TDC at roughly 12:00 as marked on the crank whilst the cover was well off, but the other spare engines I had sitting had the 12:00 pointer position which is what gave me the clue. Removing the plug from the last cylinder position in the head will give you TDC in seconds.
