Clutch throwout bearing

I'm having issues with my throwout bearing. When I use the clutch, the sleeve for the bearing turns, and gets jammed on the clutch forks, not letting the clutch release properly. I have a return spring on the bearing, and am wondering if its just an adjustment issue, or if the clutch forks are worn out? Thanks.
