According to my 2001 Ram service manual set, there are two versions of the NV4500, one is for the 5.9 liter engine and the other, a heavy duty, is for the V10 and the Cummins engine. It says, " Main differences are larger diameter input shaft, output shaft, and mainshaft fifth gear in the heavy duty model."

The manual also talks about a model NV3500 medium duty five speed transmission used in Ram. I have to admit I don't know if there is a V6 option in the Ram, but maybe it is used there.

As an aside, I have a six speed in my 2001 Cummins Ram. My experience in having both transmissions, first the five speed NV4500 with a V10 and now a six speed with the Cummins, is the five speed shifts much smoother and more easily, which is not to say the six speed is not acceptable, it is just markedly different. It seems to want a little more time to execute its shifts. In defense of it, there are some pretty large masses in there the synchronizers have to work with, and consequently they have, apparently, a bigger job. The five speed trans shifts more like a car, and the six more like a truck....
