I had exactly the same problem, but as mine is a hotch-potch of years and models, I figured that was the cause. I tried all the suggestions mentioned and came up the same, so moved the cab as far as I could, then made a tapered steel wedge to go between the box and the frame. My steering box came from a 1.5 ton 2wd, all the PW steering arms, etc fit straight up, except the dash alignment. The wedge fixed the problem.
