I believe that the reason for stepped wheel cylinder is to force the rear shoe relatively harder into the drum to compensate for the "self energizing" action of the forward shoe, so that the drum would not be cocked forward during braking. The larger bore of a stepped cylinder is pointed to the rear, so that more force is applied to the rear shoe, since force = pressure x area. It may be that the need for smaller inventories (fewer individual parts) outweighed the need for perfectly balanced braking forces within a brake drum.
