They are press fit, then reamed, and maybe honed too.

Many miles, the looseness adds up, the piston pounds on the bushing.....

The worst engine I ever took apart happened to be a 230 in a '41 WC Panel. Valves burned to look like daisies, broken rings and ring lands, circlips fell out of the pistons. But the loose circlips bouncing around had burred the edges of their grooves so bad I had to carve the burrs out with a knife before hammering the pins out. We junked it. Before tear down, it got 10 mpg. But when cold, we had to tow it to 25 mph in 2nd gear to get it to start.
