I would jack it up, disconnect the front and rear drivelines. , start it up, put it in 4th gear then get under and listen. gear related noises can be hard to isolate on these trucks sometimes. Occasionally people think the Tcase or tranny is bad but the rear diff ends up being the culprit and the noise transfers forward
