I got the bushings from Charles Talbert at M-Series when I ordered some of his custom parts, VPW probably has them too, and as Keith said you could probably find something close from McMaster and have a shop turn them to fit if needed.
I also did the shaft, though while everything Keith said is right, when I cut the old shaft loose it turned out to be smaller OD where it passed through the mounting plate on my 46, so I had to get a neighbor with a metal lathe to turn it down to fit the bracket before I re-welded it. Simple enough with the lathe but a lot harder without it.
