Absent some evidence the truck was rebuilt in the last 15 or 20 years, there is good chance this engine will need a rebuild.

Sure, do all the checks you need to reassure yourself there is not a lesser problem that can be fixed. But, most likely you're going to need to break the piggy bank and come up with about $2,500 plus a lot of time and effort to get a new engine installed.

Unless the truck is a a pretty nice, rust-free and complete truck, you might be better off expending your resources on a truck which might yield better results in the end.
