Several ways. Drill a hole in the disk, insert a sheet metal screw. Pull on the screw with vice grips, claw hammer or whatever. Slide hammer works if you have one, but not needed.
Yep, the plugs are ruined they need to be replaced anyway. The steel ones are not expensive.
It's impossible to clean the block and a waste of time if you don't remove the freeze plugs. Most of the junk has settled to the bottom of the block.
