I've also used a pneumatic hammer with good results after following, roughly, the same procedures as outlined above.
In some cases, the rivets can only be driven out easily in one direction. One side has a slightly larger diameter hole than the other. So, when they strike the hot rivet, it expands into the large hole side and locks the rivet.
You need to cut off the factory rivet head and drive towards the formed head.
