Have you tried running it with the air cleaner off? Also verify that you have no exhaust restrictions. I had a half ton that behaved in a similar fashion and I found a piece of sheet metal obstructing the exhaust flow in the muffler. At low RPM's it ran fine but under a load or at higher speeds the engine couldn't breath. Some times a mouse nest will do the same thing.
