Lots of turbo's use water cooling.
Reason the outlet points to the front is to make it simpler to set the unit up and easier to get rid of the exhaust gas (although it would still be "interesting"). There's a lot more plumbing needed if you put the turbo under the original exhaust - you might even have to run the pipe through the cab.
Basically there's two systems, one is set up like this drawing through the carb, the other blows through the carb or uses fuel injection - for this type application, the draw system is better as our carbs aren't designed to operate under that sort of pressure.
It can't push the air backwards as it's a directional pump.
