I just installed one a couple months ago, but I can't remember exactly myself. I made up my own rubber parts because I was too cheap to buy them. If memory serves, there's a sheet metal piece with slightly bent-up edges and a thinwall tube welded in the middle (had to make one of them, too). That tube goes inside the rubber, and the bent edges of the sheet metal wrap slightly over the rubber, probably to keep it from rotating. At that point, I think only the hole in the frame is big enough for the rubber tube to go into, and the hole in the housing isn't, so the rubber tube and flat piece go against the frame bracket. Now your bolt goes through the housing, hard up against the metal, through the sheet metal and rubber pieces, and then on the bottom you have another big rubber washer, your big metal washer, and your nut.

In any vibration-isolation system there should be no continual metal contact between the two pieces you're trying to isolate. A lot of guys make the mistake of putting a thick rubber washer or something like that in the assembly, thinking that will isolate the vibration, but when the nut and bolt are tightened down to the metal on either side of the rubber washer, it carries the vibration right through.

That said, on almost all the old rigs I've see, including most M37s, the original rubber insulator was lost long ago, and somebody just bolted the housing to the frame brackets. No harm seems to come of it if you don't mind the vibration. I double-checked my sno-cats, and they use their own proprietary mounts with no rubber involved at all.
