Read Marty's and Paul's comments below. This can happen under a number of circumstances. I have heard of diesels sucking in air-borne dust in graineries or sawmills. This serves as and uncontrollable external fuel source. Or it can be an internal problem such as they mentioned. But it's also pretty rare.
