Re the 2" of suitable foam insulation. I know of a couple of guys that somewhere got the centre cutout sections from new residential front exterior doors, (where they install glass rather than leaving them solid.) They leveled and compacted the gravel base, laid the cutouts down, then poured the concrete.
Like everything else you have to be ale to get them at the right price.
I know of another guy who has over 1500 of these piled in his side yard, --- ready for his project.
