Should be able to send them to the guy Ray specifies. He provides the rotor mounting plate when he modifies them.
I think I still have the instructions if you need them.
The only other pieces you'd need are the fasteners which should be easy to get, and the spacer plate for on the hub, which you'd probably have to have cut by someone with a CNC plasma or similar.
Ray provides hoses and such but they can be purchased over the counter when you get the calipers and rotors. He lists the model info in the instructions on the site.
I think I still have the brake hoses from the kit if you wanted them. I installed braided stainless lines on mine.
