If the truck you are looking at does not have a clear title, then in my mind that should cause it to have a lower price than one with a title, something to keep in mind with your negotiations. But, being able to get a clear title is the most important thing.
Each state tends to handle it a bit differently.
You might want to look at Broadway Title on- line, they list the states where they can and can't help you get a title. If Illinois is on the list of "no", then that could be a big problem.
