It is helpful to remember that rust is typically 8 times less dense than solid steel. (the expansion ratio can vary greatly, but it is always between 4 and 10)This means, even with a full 1/4" of rust (an enormous amount), it only depleted 1/32" of steel (a fairly irrelevant amount).
Generally all that is required is rust removal and cleaning, not necessarily a structural repair. The biggest concern is what is called "rust smacking" which you may see between frame-rail layers. You will need to clean that for sure.
