Cab mounts revisited

Ok, now that I have the correct wood block cab mounts coming from Mid West Mil, can someone post or email me a picture or two on how the bolts, springs and wood blocks all come together? I checked the parts list book and nothin in there other than listing them.

Mine only had one bolt and spring in place with no wood, so Im not sure how they go back. And there should be three bolts with springs correct? Drivers side front corner is bolted solid? Blocks in the back...
