That configuration is known as a Hog Ringer.

In the time before the spiral cable wrapper was pulled along the messenger strand to lash the cable to the messenger hog rings did the job.

Originally rings were installed by a man riding the messenger in a bosun chair. That required the man to dismount the chair at each pole, move the chair to the next section and remount the chair to be pulled to the next pole.

The idea was for the Hog Ringer to be able to hang more cable in a day with less manpower. 1 driver and 1 man ringing could minimally do the same footage in a day, the driver could jump in and out of the truck to pick the traveling blocks up at each pole while the man on the ladder did the transition hardware. The third man who had formerly pulled the chair along was no longer needed.

The steel ring prevented the ladder from going low enough to put the truck on 2 wheels with a 250# lineman on the end of the ladder.
