Yes you want to take the old paint off. There are several ways of doing it. Do not sand blast it in the normal sense as that process is designed for heavier metal.

Plastic:
You can use plastic media which will take paint off and not dammage anything. It will not take rust off. You can leave the windows and rubber on and it will not dammage them either. It will not take any rust off.

Garnet and walnut shells:
You need to have the truck completly taken apart. Windows trim and everything off. This is a different form of sand blasting. The garnet is not as hard on the metal and the walnut shells provide an additional buffer. It will take rust off and is very mildly agressive. It will scour out the rust and you can end up with holes where the rust was which is a good thing as you need to replace the rusted metal.

Paint Stripper:
Messy and you need a mask for fumes. It will eat through your layers of paint and then you need to scrape it off. You then need to rinse and scotch brite the metal clean. It is messy but you can do it yourself with out much problem. You will still need to attack any rust you come across.
